CONSENT

On your first attendance at the clinic you will be required to sign a clinic consent form. By signing this document, you confirm that you consent to treatment at Belper Physiotherapy Clinic. We will be unable to provide you with treatment unless this form is signed. You are free to withdraw this consent for treatment at any time and do not need to provide a reason for withdrawing your consent. Treatment will immediately cease should you withdraw your consent.

During your treatment we may ask to photograph / video your postures / movements or record exercises you need to undertake at home. If you do not consent to this, please advise your physiotherapist. These are for your personal use only and will not be shown / shared with any other parties.

We are legally required to obtain your consent regarding how we use your personal information. You can read about this under the section below that is titled

CANCELLATION POLICY

 Cancellations can be made up to 24 hours prior to your appointment without charge, please advise cancellations by phoning the clinic on 01773 829623.

Should you cancel with less than 24 hour’s notice, or fail to attend your appointment without notice then the full fee for the appointment will still be charged. This will be automatically invoiced to you and payment is due within 7 days of the late cancelled / missed appointment.

Please note that if your treatment is funded via a private medical insurance policy or via a medico-legal claim, then we are unable to invoice the funding party for these costs. This means that you are liable for the fees of any late cancelled / missed appointments.

CHAPERONE

Belper Physiotherapy Clinic Physiotherapy is committed to providing safe and effective care, in a comfortable and welcoming environment. Should you wish to be accompanied by a chaperone then please feel free to do so. Anyone receiving treatment under the age of 18 is required to attend with a parent or guardian.

REQUEST FOR MEDICAL RECORDS

Requests for copies of medical records must be made in writing by either the client, the instructing / funding party or their legal representative. Any party requesting copies of medical records other than the client, must include a signed authority form, that authorises the release of the clients records to that specific party. The signature on this form must match the signature that we hold on file. This form must be compliant with the General Data Protection Regulations (GDPR) 2018. Records will not be released in the absence of this document. We reserve the right to ask the client attends to confirm their identify (passport, drivers licence,) before notes are released.

All medical records requests are handled in accordance with GDPR. In certain situations we may charge a fee for the processing of these requests. If a fee is required, notes will be released once payment has cleared.

If you are the client, you can request to view your notes and personal information within the clinic free of charge, this will be during your appointment time. You are also able to view copies of letters and reports that are about you, before they are sent off.
